Robertus M.A. de Bie, MD, PhD is a Professor of Neurology at the University of Amsterdam, Academic Medical Center, in Amsterdam, Netherlands. Susan H. Fox, MBChB, MRCP(UK), PhD is an Assistant Professor of Neurology at the University of Toronto, Toronto Western Hospital in Toronto, Canada.
