Fox, MB ChB, MRCP(UK), PhD, is Associate Professor of Neurology at University of Toronto, Movement Disorders Clinic at Toronto Western Hospital. Robertus MA de Bie, MD, is a neurologist, Department of Neurology, Academic Medical Center, Amsterdam, Netherlands.
