Uscito a puntate, dal 21 febbraio al 6 aprile 1869, sulla rivista milanese Il Pungolo e pubblicato in volume nello stesso anno, il romanzo si ispira a fatti autobiografici vissuti dal Tarchetti nel biennio '64-'65, quando conobbe e frequentò, tra Milano e Parma, la giovane epilettica Carolina C., la quale gli suggerì la figura di Fosca. L’opera non fu terminata dal Tarchetti a causa della sua morte prematura ed il 48esimo capitolo, l’ultimo, venne scritto da Salvatore Farina, suo amico e consigliere.
